MPF QC Documentation Checklist Updated
Effective Date: Immediately (unless otherwise noted within)
The MPF Program is updating the MPF QC Documentation Checklist and converting it to be “Form OG4: Quality Control Checklist.”
Effective February 10, 2025, PFIs will be required to upload the completed Form and upload all applicable documentation, to eMAQCS®plus to comply with MPF Quality Control post-closing review requests. Please note that emailed documents will not be accepted.

In addition, the MPF Program Guide has been updated to add references to the From. See MPF Program Guide Section 8.1.
As a reminder, only individuals identified in eMAQCSplus will receive MPF Quality Control requests and notices, and only individuals with eMAQCplus access are able to upload responses to the system.
To update email contacts for MPF Quality Control file reviews and results, visit eMAQCSplus page on the FHLBMPF webpage.
Individuals needing access to upload files to eMAQCSplus should contact their institution’s Security Administrator through eMPF.
If your PFI does not have a Security Administrator or you are unsure who your [security admin] is, please contact your FHLBank.
If you do not have a [security admin] and would like to have an individual at the PFI be able to assign credentials, receive MPF Quality Control Department requests, notices and upload responses to the eMAQCS portal, please contact your FHLBank.
If you have further questions or encounter any issues during the login or registration process, the MPF Service Center can be reached at MPF-Help@fhlbc.com or (877) 345-2673, select option 6 – this is a dedicated menu option to further assist through the registration process.
As part of our ongoing efforts to maintain security and access control for eMAQCSplus, we are reminding you to contact the MPF Service Center promptly whenever a team member with access leaves the organization or moves to a role that does not require access. All individuals must have their own log-in credentials. It is crucial to deactivate their credentials to safeguard sensitive information and systems.
