Update to Exhibit Z-X MPF Xtra Flex Modification (FLEX) Request

Effective Date: December 1, 2024

Effective December 1, 2024, the MPF Program is updating the name of the form used to submit requests for FLEX modifications in eMAQCS.  The name will change from “Evaluate-Flex Modification (FLEX)” to Evaluate-Flex Modification (FLEX2024), but everything in within the form and all required inputs will remain the same.

MPF Guide references to the form will be updated at a later date.
